Appointment
Physical Therapy is provided on
the written referral of licensed
physicians, dentists, podiatrists,
nurse practitioners and physicians's
assistants.
Contact our office at 631-1212 to
schedule an initial appointment. Day
and evening hours are available.
Patients are asked to arrive early
for their initial visit to complete
the necessary paperwork. Additional
appointments will be scheduled by the
receptionist at the conclusion of
your initial visit.
What to Bring
It is important to have the following
with you on your first visit:
• A written prescription
• Non-restrictive, comfortable
clothing and footwear
• Completed insurance
information including address,
identification numbers and referral
if indicated by your insurance
company.
Visit Frequency & Length
Please allow 60-90 minutes for your
initial visit. The length of
subsequent visits will depend on the
nature and intensity of service
necessary in your case.
Payment for Services
Because our services are provided on
the written referral of a qualified
healthcare provider, most insurance
companies will honor your claims. Our
staff will be happy to verify your
coverage for services at or before
your first visit.
